Introduction
ADP-Mbozi is a registered national NGO with registration number 1639. The organization has
been in partnership with Pact Tanzania in implementing The Adolescents and Children HIV
Incidence Reduction, Empowerment and Virus Elimination project, or ACHIEVE, is a five-year
global effort to reach and sustain HIV epidemic control among pregnant. and breastfeeding women,
adolescents, infants, and children. The project is funded by the President’s Emergency Plan for
AIDS Relief (PEPFAR) through the United States Agency for International Development (USAID)
and implemented by a Pact-led consortium of top global HIV/AIDS partners, including Palladium,
No Means No Worldwide, and WI-HER.
ADP Mbozi through ACHIEVE project, will provide ART Calendar to 7265 CLHIV and
educational subsidies contained School bags, Mathematical sets, Pens, Pencils, and A4 Exercise
books for 15,680 adolescent girls aged 10-14.

Drafted inception explaining the methodology to supply the items
Quote, Valid for at least 90 days
Current company files
Copies of Office registration certificates and address, valid business licenses, VAT, TIN
certificates, and Tax clearance.
Items of specification are explained in the above table.
Current audited Financial Statements for two consecutive years for the supply of education
subsidies.
Original bank statement for the past 12 months for the printing of ART Calendars.
Delivered time must be specified.
Payment terms 100% after delivery.
Evidence for similar items/services and at least two (2) names of clients served with their
addresses.

How to submit:
Each LOT should be submitted separately in an enclosed and sealed envelope.
The eligible vendors should physically submit their proposal to Tender committee, P.O.
Box 204 Mbozi, Songwe-Region
All quotes must be in Tanzanian shillings and enclosed in a plain sealed envelope.
The deadline for proposal submission will be 14 December 2022, 10:10 AM, and ADP
Mbozi will open the bids on 14 December 2022 11:00 AM
